What Are Home and Community-Based Services (HCBS)?

Home and Community-Based Services (HCBS) are available in all fifty states across the United States. This federally funded Medicaid program provides long-term services and supports (LTSS) that allow individuals to receive care in their own homes and communities rather than in institutional settings.

Funds are distributed through each state’s Managed Care Organizations (MCOs) and waiver programs. The goal of HCBS is to enhance quality of life by supporting independence, dignity, and inclusion for individuals who require long-term care.

Who Can Benefit From HCBS?

HCBS serves a wide range of populations, including:

  • Individuals with developmental and intellectual disabilities

  • Individuals with mental health conditions

  • People with physical disabilities

  • Individuals living with HIV/AIDS

  • People with Autism Spectrum Disorder

HCBS in Pennsylvania

In Pennsylvania, there are currently 12 HCBS programs. Each program has its own requirements and criteria. To qualify, applicants must meet both income and health guidelines, which vary depending on the program. Importantly, eligibility requires being considered “nursing home eligible.” This does not mean you must enter a nursing home—it simply means your health status requires long-term care. HCBS makes it possible to receive that care in your home and community instead.

Services provided through HCBS can be tailored to meet individual needs. Examples include:

  • Employment Services (job search, job coaching, and retention support)

  • In-Home and Community Supports

  • Personal Assistance Services

  • Companion Services

  • Respite Care

  • Community Participation Supports

  • Skilled Nursing

  • Therapies (physical, occupational, speech, etc.)

  • Behavioral Supports

  • Assistive Technology

  • Home and Vehicle Accessibility Adaptations

  • Transportation Assistance

  • Life Sharing Programs

  • Supported Living

  • Adult Day Services

  • Home Health Care

  • Counseling

  • Specialized Medical Equipment and Supplies

  • Environmental Modifications

  • Home-Delivered Meals

  • Personal Emergency Response Systems (PERS)
