• Wellness Calls: Scheduled phone or video check-ins to ensure clients are safe, comfortable, and have what they need. We also offer text and telephone calls.

• Home Visits: In-person visits to provide companionship, monitor living conditions, and identify potential concerns.

• Medication & Appointment Reminders: Gentle reminders to help clients stay on track with their health and daily routines. This can be done through virtual, telephone and texts.

• Family Updates: Clear communication with family members or caregivers to keep them informed about the client’s status and needs.
